DescriptionComprising a series of numbered statements to be put before the subject, concerning his attitude to the book of Common Prayer, his religious practices, relationships with unlicensed preachers, reading of banned texts, failure to baptise his children etc.

Item 5 states that the unnamed subject did at "divers and sundry times .... have procured one [blank] Wright, being a layman and not being admitted unto any orders by any Bishop of this Realm... to say divine service, minister the Sacrament, catechize, to preach, to read and to interpret upon the scriptures, within the house of the above named Lord Rich at St Batholemew within the City of London, at Leez and at Rochford within the county of Essex and within the house of one Mr Butler of T... and at divers other places within the said county".

Mentions one Greenway, schoolmaster at Felsted; Bingham, an official to the Archdeacon of Essex; and Barwick "a godlie preacher".

Undated but refers to the "Sovereign Lady the Queen" throughout.
